We are currently accepting new patients, please fill out the form below or give us a call (207)-846-9499! After scheduling your initial appointment, we will email you new patient forms. We kindly ask you fill out your initial paper work at least 24 hours prior to your first appointment. It allows Dr. Coffin time to take a look at your file, and plan the best treatment plan for you to get the most out of your first appointment! If the paperwork is not filled out in advance, we will have to reschedule.
